The best route depends on what day of time you travel. I recommend learning to plan with Hyperdia this way you can find any route/timetable and more. It's a great tool for planning with the JR Pass. We also have a useful interactive map with all JR lines included.

When it comes to what kind of pass to use, I think your best option will be to look at a 7 day JR Pass or buy normal tickets. There is no other option that covers all places you want to travel to. A Hakone Free Pass is recommended for Hakone.
