Which Pass and is it going to be worth it

I'd say you're pretty close to a 14-day JR Pass, but as it stands would have to add a little but of travel. Maybe a day trip or two would be enough. I'd also consider it for the convenience, as it covers travel between all the cities you plan to visit.

Note that within Hakone, you may still need additional transport.

I have reviewed your trip (again) and added up all the fares for your trip.

While the difference is fairly small, it would cost less to buy normal tickets here - rather than getting a 14 day Japan Rail Pass. You could still consider it for convenience, as it saves you from having to buy tickets all the time but if it's real budget that's the concern, then I'd stick with normal tickets.
