How do I apply for reissuance @ reprinting of my trip using Fresh-Hitachi (Ueno-eki to Katsuta-eki)? My mistake was I lost the receipt and I need it for claim purpose because the trip was borne by my company. Now I am already in my country and hope I can get them back online, if it's possible.

Unfortunately this is something I can not help you with. Possibly you could use a statement on your Credit card if a available. Otherwise I am afraid that there's not much to be done.
For reference: the fare is ¥ 4,330.
