We have a question about how to fill out the first name field for the pass order form. Our US passports list both the first and middle name under "given names." The order form only has first and last name fields. Should we include the middle name in the first name field then? If we don't include the middle names will that be a problem?

Hi Tracy,
The most important thing is that your first and last names matches your passport. So, you do not have to include any middle names.
