That's a nice selection of places you plan to visit in the Nagano region. You can visit Kamikochi, Kiso Valley easily from Matsumoto city. While Yamanouchi, Shibu Onsen, and Nozawa are more accessible from Nagano city, as it is on the Hokuriku Shinkansen.
I think it makes sense to make Matsumoto and Nagano your two main bases and plan around that. Then either make day trips or stay a little longer in places you really like. I recommend reading up on the Hokuriku Shinkansen line as you will use it to access most places from Nagano. For example, you can use Iiyama station to access Yamanouchi, Shibu-onsen and Nozawa.
A full itinerary would depend on how many days you have, what places you have most interest in and the availability of accommodations.
