Does the JR pass include enough Train lines to go around Tokyo comfortably?

For 4-5 days in Tokyo, is the JR pass enough or should I get some sort of subway pass? And for Kyoto and Osaka?

It completely depends on where you go, how many trains you travel on and so forth. Without knowing the details of your trip, I can't answer this.

It would be to travel around Tokyo to Shinjuku, Akihabara, Saitama, Shibuya, all those places. And then other more specific places like Ghibli Museum, Kirby Café, Teamlab Planets, Tsukiji Market...etc
