I just bought and received a exchange order for a JR Pass. However, I forgot to put in my middle name and it isn't shown on the order. As my passport shows my middle name, but the order doesn't, will this be a problem when exchanging for a JR Pass in Japan?

Generally we don't need a middle name in order to buy the JR Pass.

It should not be a problem - exchanging a voucher is not the same as an airline ticket, where Immigration is involved as well as terrorist watch lists etc. As long as the rest of your name is correct, getting the pass should be smooth and quick.
