Has been reading through the JR line and Shinkansen line but very confusing and we have no idea if this itinerary work or not.
Can someone please kindly advise if it possible for the itinerary above? Or, we should revert the trip start from Tokyo instead of Osaka?

In my opinion it makes the most sense to do your itinerary as follows:
Tokyo --> Yuzawa --> Tokyo --> Hakone --> Osaka --> Kawaguchiko --> back to Osaka to fly out.
It allows you to base yourself in Tokyo/Osaka so you can access Yuzawa/Kawaguchiko (perhaps each for day trips or one night each, but this depends on how long your itinerary is) as they will be closest major cities in your itinerary. You can then fly into Tokyo and leave from Osaka.
