My daughter is 11 years and 10 months. On the date of our trip to Japan she will be 12. Is she eligible for a Child Pass if the exchange order is issued before she turns 12?

That's the official rule book so yes you can assume it is right!
It states as of the date on which the exchange order is issued, this is the moment when the JRP order is processed and printed. I'd advice ordering in at least 2 weeks in advance of her birthday to avoid possible problems.
