Hi,
The first thing that is apparent is that you don't need a second 7 day pass - you wouldn't save anything on it anyways. Just get the 21 day pass and use it for the last 21 days of your trip. For your Day 1, if entering through Narita, you can use the 1000 yen bus. For Tokyo, you might consider a 2 or 3 day subway pass.
To see Nikko, you can use a Tobu Nikko Free Pass, and for Kamakura/Enoshima, there is the Enoshima-Kamakura Freepass. For Koyasan, use the Koyasan World Heritage Ticket.

Hotels are generally cheaper in Osaka than Kyoto, but there are some good cheap places in the latter, and it would take you an extra hour round trip to go from Osaka to Kyoto every day.

Visiting both Fukuoka and Nagasaki are worth it; but if you choose to do the latter as a day trip, it'd take 2 hours one way.
For Kagoshima, don't miss seeing Sakurajima and the Senganen Garden.

Beppu is famous for its hot springs, but its Hells Tour, plus the Takasakiyama monkey park/aquarium are all terrific.

Your last days need to be firmed up a bit. Kamikochi and the Kiso Valley are both time consuming to get to.

Good luck.
